Registration Information

To register your child at St. Patrick Preschool for the 2024-2025 school
year, please complete and submit the following:

• New Family Registration Form
• Student Registration Form
• Copy of your child’s birth certificate
• Copy of your child’s baptismal certificate (if applicable)
• $150 non-refundable registration fee (via cash or check made payable
  to St. Patrick Preschool)

You may submit these forms in one of the following ways:

• Drop forms off in the lockbox outside the front entrance of the
  Preschool building
• Mail the forms to the Preschool Office (118 N. 5th St., St. Charles,
  IL 60174)
• Scan and email the forms to preschool@stpatrickparish.org
• Fax the forms to 630.338.8208

 

Important Health Information

Illinois state law requires all students entering Preschool for the first time are required to have a current physical and be up to date on required immunizations. St. Patrick Catholic School has the right, according to state regulations, to exclude a child from school if documentation is not turned in timely, until such time as the child presents proof of having a health examination and/or current record of all required immunizations. Health exam must have been completed within one year prior to school start date.

• Physical exam signed and dated by doctor
• Immunizations signed and dated by health care provider
• Medical history filled out and signed by parent/guardian
• Forms must say Rev 02/15 or Approved SHP with date if electronic
